Description

The contract for the provision of a Dementia Support Service is from 01.04.2021 for an initial period of 3 years until 31.03.2024 with the possibility to extend for a further period(s) of up to 2 years (31.03.2026), taking the contract term to a potential maximum of years. This service aims to assist people living with dementia and their families/informal carers in Leicester and Leicestershire. It is designed to be flexible and proactive in delivering personalised, strengths-based community support, relevant to people’s circumstances and the stage of their condition. The support delivered by the service will fall under one or more of the following categories: — information, advice and signposting to other services, — personalised targeted support, — post diagnostic group and social group support, — informal carer learning.
