Description
The aim of the Service is to prevent heart disease, stroke, type 2 diabetes and kidney disease, raise awareness of dementia both across the population and within high risk and vulnerable groups. The provider will deliver an NHS Health Check programme that will maximise the number of NHS Health Checks delivered, within the available resources.
