Description
The Stroke Recovery Service (hereinafter referred to as 'the Service') is provided by Stockton-on-Tees Borough Council and supports people who have survived a stroke to receive support, information and social interaction following their discharge from hospital. It is an element of the Acute Stroke Pathway in Stockton on-Tees, which also includes the Community Stroke Team (CST) commissioned by the North East and North Cumbria Integrated Care Board (ICB) as part of the Community Integrated Assessment Team (CIAT). The CST provides rehabilitation services, including physiotherapy, occupational therapy, speech and language therapy, and makes referrals for specialist therapies where appropriate. The Service will be available to all stroke survivors who live in the Borough of Stockton-on-Tees and who are registered with a GP in Stockton-on-Tees.
