Description
Islington Council procured a Housing Related Floating Support Service to support vulnerable residents to maintain their tenancies and live independently in the community. The service is designed to prevent homelessness and reduce the need for more intensive interventions by providing time-limited, person-centred support to individuals at risk of tenancy breakdown. The service includes a dedicated hospital discharge pathway to support timely and safe discharges from hospital into independent accommodation, as well as support for individuals subject to Multi-Agency Public Protection Arrangements (MAPPA). The provider is expected to work in close partnership with health, housing, and social care professionals to deliver a coordinated and responsive service.
