Description

The service provider will be expected to work with Key stakeholders across ophthalmic services within the central and West integrated health community, including all ophthalmology patients, service users and families/carers. Collaborative working within the Multi-disciplinary team (MDT) will be a key aspect of this role. The service provider is expected to provide: -Provide advice, information, practical and emotional support, to people of all ages affected by sight loss. -Provide a person-centred screening of needs, signposting and referring patients to a wide range of services and organisations -Cultivate positive working relationships to establish and sustain referral routes with a range of health, statutory and third sector organisations. -Provide basic visual awareness training to eye clinic staff and others as required. -Act as a key contact to support patients, family members and carers to navigate their way through the variety of information, advice and support available. -To …
