Description
This specification should be considered as part of the wider homeless health pathway. The service provider is expected to work in partnership with homeless agencies (as locally commissioned) as well as a range of health, social care and voluntary services to ensure a seamless and equitable service for homeless patients. The service provider shall: • Provide clinically led outreach clinics across multiple sites located in a community setting providing primary care health services to the homeless population. • Designated sites have been identified through collaborative engagement across SEL ICB, LBL colleagues (Housing teams, rough sleeping teams, health inclusion teams) and housing providers.
