Brentworth PCN £3,779,950<br/>2. Chiswick PCN £2,458,579<br/>3. Feltham and Bedfont PCN £5,342,688<br/>4. Great West Road PCN £4,213,421<br/>5. Hounslow Health PCN £5,933,308<br/>- the dates between which these services are to be provided is from 01 April 2026 to 31 March 2028 (24 months). (Note that this is a retrospective start date).
