Description
The aim of this service is to aid General Practice and patients to cope with the surge in demand so as to reduce the complexity of current access arrangements to urgent care for patients and move towards a single integrated approach to integrated urgent care to guaranteed same day access for patients. Pathways will extend core and enhanced access primary care provision, provide easier access to primary care clinical support prior to unscheduled care and support better planned care. This will allow General Practice to continue to lead a system that educates patients, reduces unplanned attendances and reduces hospital admissions and meets the reasonable needs of their patients lists.
