Description
The City of York Council Public Health service are seeking to engage with suitable suppliers to procure access to a NHS Health Check service for eligible citizens aged 40-74. To be eligible for an NHS Health Check, citizens must live within the City of York Council area, be aged 40-74. Those with an existing cardiovascular or metabolic condition (e.g. diabetes or high blood pressure) are excluded, as the aim of the programs is to find undiagnosed and treatable conditions. Citizens with existing long-term health conditions should be receiving on-going care and condition management from their healthcare professional. The NHS Health Check comprises a standardised set of risk assessments.
