Description
The overall contract aim is to support adults, children and young people to: • Develop realistic, healthy and sustainable habits in relation to food, physical activity and general well-being. • Maintain or lose weight. The service has four core objectives 1. Deliver a service that supports adults to adopt healthier behaviours and to set and achieve personalised and realistic goals regarding their weight and overall health (As per the specification for the Holistic Adult Weight Management Service in section 4 which includes sub-contracting responsibilities). 2. Temporarily support any remaining service users on the adult Tier 2+ pathway (As per section 5) 3. Deliver a service that supports children, young people and their families to adopt healthier behaviours and to set and achieve personalised and realistic goals regarding their weight and overall health (As per the specification for the Holistic Family Weight Management Service in section 6).
