The service will deliver three levels of support: Universal Offer includes community-based health promotion, healthy eating guidance, physical activity initiatives, emotional wellbeing support, and signposting to local resources. It aims to build health literacy and encourage self-care strategies. Targeted Support provides structured one-to-one and group programmes for individuals needing more intensive help, including rehabilitation for long-term conditions and behavioural interventions linked to NHS Health Checks. Specialist Services focus on evidence-based interventions for weight management and smoking cessation. Weight management support will follow NICE guidance, offering structured behavioural programmes for adults with a BMI of 25-39.9 (or 23+ for ethnic minority groups).
